Our 245D Waiver Services

We provide compassionate, person-centered care to help individuals live safely and independently at home.

Night supervision

Provides overnight assistance and monitoring by staff in the person’s own home.

24-hour emergency assistance

On-call counseling and problem-solving and/or immediate response for assistance at a person’s home due to a health/personal emergency.

Homemaker services

Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.

Respite care services

Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Individual Community Living Supports (ICLS)

For people who need reminders, cues, intermittent or moderate supervision, or physical assistance to remain safely in their own homes.

We provide all three categories of Individualized Home Supports to meet each individual’s needs:

Without Training

Individualized Home Supports without training are provided to adults or children who need support, assistance, or supervision in at least one of the community living service categories while living in their own home or family home.

With Training

Individualized Home Supports with training are provided to adults who need support and training (habilitation) in at least one of the community living service categories while living in their own home or family home. These services focus on developing skills in areas such as community participation, health, safety and wellness, household management, and adaptive skills.

With Family Training

Individualized Home Supports with family training are provided to adults or children living in their family’s home when the individual and/or their family need support and training (habilitation) in at least one of the community living service categories. These services also focus on building skills in community participation, health, safety and wellness, household management, and adaptive skills.

Frquently Asked Questions

Are “245D services” the same as a waiver?

245D is the licensing framework for certain HCBS services. Many of these services are funded under Minnesota Medicaid waiver programs.

Is 24-hour emergency assistance the same as calling 911?

No. DHS states it does not replace 911 when necessary.

Are these services always available immediately?

Start dates depend on authorization, staffing, and program requirements.
